Terms and Conditions
Payment
You will receive an invoice after each appointment. Invoices will be sent via email. Payments should be made within 7 days of the date of the invoice for individual clients or according to the payment terms of the education setting. Payments can be made via online banking. I prefer not to take cash payments and I do not have card facilities. If you have any difficulties meeting the costs of therapy, please talk to me about this as soon as you are aware of this. If your payment is missed or not made on time my services will pause immediately. Parent/ carers using private healthcare are responsible for settling the invoice and claiming from the insurer concerned.
Please inform me of any changes to personal details, inclusive of home address, contact telephone number and email addresses so that I am able to provide you with current, updated relevant information.
Cancellation policy
I require a minimum of 24 hours’ notice in order to cancel an appointment and we can rearrange the appointment. Please contact me via telephone or email in order to cancel sessions.
Same day cancellations, apart from illness, will be charged at 50% of the therapy fee.
If I need to cancel an appointment, the maximum possible notice will be given and the appointment will be rescheduled the next possible convenient time.
Please inform the therapist if the child is unwell and is not going to present for the therapy session either at home or in nursery.
Electronic Notes
I will keep confidential case notes about your child’s progress, in order to plan therapy and monitor progress. I will keep the information I hold about your child up to date, safe and secure, in line with my responsibilities under the Data Protection Act.
By using my service, you agree that I may communicate with you electronically regarding all matters to your child’s care. I may also communicate with you regarding security, privacy and administrative issues relating to your use of the services.
I will keep your child’s notes on an encrypted hard drive. My computer is password protected. Any reports sent electronically will be password protected and will be sent to you by email. The password for the reports will be sent via an alternative means of communication i.e., text message. If e-mail is used as a means of communicating it is advised that you ensure that all e-mail messages are properly addressed. Please be aware that the confidentiality of e-mail sent via the Internet cannot be guaranteed.
Sharing of information
It is important for your child’s care that I liaise with other professionals and share relevant information when necessary. All reports will be sent to parent/carers and I would ask that they distribute the reports to relevant professionals involved (Education settings, Doctor/Paediatrician, Health Visitor). I will ask permission before I speak to other professionals involved in your child’s care.
If another Speech and Language Therapist, whether NHS or private, is involved in your child’s care, it is good practice that we liaise and work collaboratively. This will ensure the most effective therapy plan is carried out. It is parents/carers responsibility to inform me of involvement from another therapist or other relevant professionals.
Disclaimer
I strive to deliver high quality care and all therapy techniques, to my knowledge, are evidence based and are delivered in order to improve the speech, language and communication skills of the individuals I treat. The services I provide may not lead to a cure and I cannot guarantee the communication difficulty will improve, and I cannot be held responsible for therapy, advice and/or programmes being carried out incorrectly by other people.
Terms and Conditions
I reserve the right to update Terms and conditions at any time. If changes to the Terms and Conditions are necessary, I will notify you via email address provided. You will be given 30 days’ notice to any changes to Terms and Conditions which directly affect the payment rates for my services. If you continue to use my service following notification of the changes to the terms and conditions, I will assume you have accepted these changes.
Memberships and Registrations
HCPC
I am a member of the Health Care Professions Council (HCPC). The HCPC protect the public by setting standards for professional education, training and practice and keep a register of professionals who meet their standards. They will take action if professionals on their register do not meet their standards.
RCSLT
I am a member of the Royal College of Speech and Language Therapists. The professional body for Speech and Language Therapists in the UK.
ASLTIP
I am a member of Association of Speech and Language Therapists in Independent Practice.
Termination of Therapy
It is within the clients’ and therapists’ rights to terminate therapy or intervention with 24 hours’ notice.
Complaints
I aim to provide the best quality care. If you believe that you have not been provided a service delivered with an appropriate level of care and skill, please contact me to discuss this at the earliest opportunity. Please discuss any complaints or concerns that you may have with me in the first instance. If I am unable to resolve the difficulty, please contact the HCPC using the information on their website.
